Very interesting! And discouraging to know the EHR systems are so flawed that they kill people, because lab results are incomplete, or delayed, or overlooked by doctors, or “The process of linking the correct medical record to the correct patient—a seemingly basic task at which the systems, even when made by the same EHR vendor, often fail”
