Do doctors take your blood so they can do 23andMe ancestry tests? Or do you have to do that on your own time?

23andme does not use blood for their tests.

Some doctors will agree to take blood for non-medical DNA analysis but will charge for doing it.

The services that rely on blood analysis can often recommend a clinic close to you where the blood sample can be drawn, though I think most commercial DNA/ancestry sites use cheek swabs rather than blood samples.

Most research groups or companies send you a kit. You’ll swab your own saliva and mail it back to them.
